Did you create Excalibur on the alchemy page? Holy Avenger is used up to create Excalibur.

Ever since Alpha Memph was release together with the Soulforge alchemy, you can get another Holy Avenger from the Land of Earth if you have Excalibur or Soulforge. However, it's still only one Excalibur and one Holy Avenger at one time.
